What is Latitude?

Latitude is a free, voluntary, and confidential personalized wellness and health improvement program designed to promote a culture of health by improving healthy habits, attitudes, and behaviors; increasing physical activity, and making positive and healthy lifestyle choices.

Ready to take your first little step?

  • INSIGHT Health Assessment: Health plan members are asked to complete a Health Assessment. You will notice an improved look and feel with the updated INSIGHT Health Assessment. You and your covered spouse should complete the assessment 30 days from date of hire.
  • Health Advising: Once you complete the Health Assessment, your Health Advisor will contact you by phone or email. The advisor will review your Health Assessment with you and offer the option of Health Coaching.
  • Condition Management: Members with chronic health conditions may be eligible for Asthma, Back Pain, Diabetes, and Heart Disease programs. If you are eligible, HealthFitness will contact you with more information.
  • Health Screenings: The Health Screening, which lasts about 30 minutes, includes Blood Pressure, Total Cholesterol, High Density Lipoprotein (HDL), Total Cholesterol/HDL Ratio, HbA1c (helps determine risk for diabetes), Body mass index, Carbon monoxide, and Waist to Hip Ratio.
  • Our Healthy Baby: The purpose of this program is to provide support to expectant parents through educational materials, supportive contact, and individual case management throughout the pregnancy.
